Collaborating with an interdisciplinary team to develop patient care plans. Monitoring and documenting changes in a patient's emotional and social well-being, reporting these to the team and the attending physician as needed. Providing valuable support and consultation to other healthcare team members on social and emotional factors impacting patient health. Connecting patients and families with essential community and family resources. Serving as an advocate, helping navigate healthcare systems, and assisting with discharge planning. Observes, reports, and uses electronic medical record (EMR) system to document patient status and the care or service furnished. Other duties as assigned.

Requirements for this role:

Qualified social workers must have a master's or doctoral degree from a school of Social Work accredited by the Council on Social Work Education and at least one year of social work experience in a healthcare setting, passing an ASWB exam. Licensed Social Worker (LSW) from a Bachelors of Social Work (BSW) education OR Licensed Independent Social Worker (LISW) Related education from a CSWE-accredited social worker program. Must have a criminal background check.
