This role offers an excellent opportunity to contribute to a compassionate healthcare environment, ensuring smooth transitions and effective communication with our clients and team members.
Key Responsibilities:
Assist the Director of Care Transitions with coordinating home health services and hospice referrals Issue notices and communicate effectively with guests and their families Maintain accurate and organized filing systems for all documentation Ensure attention to detail in all tasks to support seamless care transitions Support administrative tasks related to care coordination and documentation Collaborate with healthcare providers and team members to facilitate smooth service delivery
