Certified Home Health Aide (CHHA)
Department:
Hospice Shifts Available:
10 hours/ week (Part-time), Schedule will be contingent on the needs of the patients
Pay Rate:
Based on experience
Essential Job Functions and Responsibilities:
Follows written instructions for patient care as prepared by an RN. Assists patients with personal care (which includes lifting and transferring patients without assistance) to preserve good personal hygiene while maintaining a healthful, safe environment. Performs simple procedures as an extension of therapy services, such as ambulation, ROM, and exercise. Assists in administering medications that are ordinarily self-administered; prescribed under written direction of Physician or as directed by hospice nurse, as permitted by State regulations. Reports changes in patient's condition and needs to Supervisor or RN. Maintains record of services performed and of apparent condition of patient. Establishes and maintains a good working relationship with the patient/family and co-workers of the Hospice. Participates in patient care conferences to provide input requiring continuum of patient care. Performs household services essential to health care at home. Good time management.
Qualifications:
Must be CNA- LTC and Home Health Aide (HHA) certified. Current CPR certified. Frequent lifting & moving. Must have the ability to lift, move, & transfer 100 lbs with the physical capability to perform all job-related duties. Valid drivers license and reliable transportation with current state minimum vehicle insurance coverage. Willingness to enforce health and safety standards Supportive and compassionate
Education:
High school diploma or
GED Experience:
1 year preferred. Successful completion of training and competency evaluation program or competency evaluation program or nurse aide training and competency evaluation program approved by the state without a contiguous 24 months lapse.
