In this role, you will provide compassionate, person-centered support in a residential treatment home. You will assist residents with daily living skills, medication routines, community integration, crisis prevention, and progress toward their individualized treatment and recovery goals. The ideal candidate is patient, dependable, trauma-informed, and committed to protecting each resident's dignity, privacy, rights, and personal choices. Essential Responsibilities Provide person-centered, trauma-informed support to adults experiencing serious and persistent mental illness. Build professional, respectful, and supportive relationships with residents while maintaining appropriate boundaries. Assist residents with activities of daily living, including personal hygiene, grooming, dressing, laundry, housekeeping, meal preparation, and maintaining a healthy living environment. Teach and reinforce independent living skills, coping strategies, communication skills, social skills, budgeting, and community-living skills. Support residents in following their individualized service plans, treatment plans, behavioral support plans, and recovery goals. Observe residents' physical, emotional, and behavioral conditions and promptly report significant changes or concerns to the administrator, clinical team, or appropriate healthcare provider. Recognize early warning signs of psychiatric distress and respond using approved de-escalation, crisis-prevention, and safety procedures. Provide medication assistance or administration according to physician orders, facility policies, approved training, and applicable regulations. Accurately document medication administration, refusals, errors, side effects, and other medication-related concerns. Complete timely, objective, and accurate documentation, including daily progress notes, incident reports, medication records, shift summaries, and other required records. Support residents during medical appointments, behavioral health appointments, community activities, shopping, recreation, and other scheduled outings. Provide safe transportation when assigned and follow all facility vehicle and transportation procedures. Prepare nutritious meals and snacks according to approved menus, dietary needs, allergies, preferences, and physician orders. Encourage resident choice, independence, community participation, and meaningful daily activities. Follow residents' safety plans and promptly respond to emergencies, behavioral crises, suspected abuse, medical concerns, or potential self-harm. Maintain confidentiality and comply with HIPAA, mandatory abuse-reporting requirements, resident-rights rules, and organizational policies. Complete shift-to-shift communication and accurately account for residents, medications, keys, funds, and other assigned items. Maintain a clean, safe, and therapeutic residential environment by following infection-control, sanitation, and emergency-preparedness procedures. Participate in staff meetings, supervision, required training, fire drills, emergency drills, and quality-improvement activities. Perform other duties reasonably related to resident care, safety, treatment, and residential operations. Minimum Qualifications Must be at least 18 years old. High school diploma or GED required. Previous experience in a mental health residential program, group home, behavioral health setting, assisted living facility, or similar care environment is preferred. Experience supporting adults with serious mental illness, trauma histories, substance-use disorders, intellectual or developmental disabilities, or co-occurring conditions is preferred. Ability to communicate respectfully and effectively with residents, families, coworkers, healthcare providers, and community partners. Ability to remain calm, professional, and supportive during psychiatric symptoms, behavioral escalation, or emergency situations. Strong observation, documentation, organization, and time-management skills. Ability to maintain professional boundaries and protect residents' confidentiality, dignity, and rights. Ability to complete medication-administration training and safely follow medication orders and documentation procedures. Ability to perform the physical duties of the position, including standing, walking, bending, lifting, and assisting residents when necessary. Must pass all required criminal-background checks and meet applicable state and organizational employment requirements. A valid driver's license, acceptable driving record, and proof of insurance may be required when transportation duties are assigned. Required or Preferred Certifications Current CPR and First Aid certification or the ability to obtain certification within the required timeframe. Mandatory abuse-reporting training. Approved medication-administration training. Crisis prevention, intervention, and de-escalation training. Infection-control and bloodborne-pathogens training. Additional training required by the facility or applicable Oregon regulations. Preferred Skills and Characteristics Compassionate, patient, reliable, and respectful. Understanding of trauma-informed, recovery-oriented, and person-centered care. Knowledge of mental health symptoms, crisis warning signs, and positive behavioral supports. Ability to encourage independence without completing tasks residents can safely perform themselves. Ability to work independently while remaining an effective member of the residential and clinical team. Willingness to work assigned days, evenings, overnight shifts, weekends, and holidays.
