Duties Assist residents with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals promptly. Support medication administration following HIPAA guidelines and care plans to ensure safety and compliance. Prepare nutritious meals and assist with meal planning, cooking, and feeding as needed while maintaining cleanliness standards. Provide patient personal care assistance with compassion, patience, and respect for residents' resident's rights. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning resident areas and ensuring infection control protocols are followed. Document care activities accurately in compliance with HIPAA regulations and facility policies; communicate effectively with team members regarding resident needs and progress. Skills Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Strong knowledge of dementia care, Alzheimer's care, memory care, autism support, intellectual disabilities support, neurodevelopmental disorders, and developmental disabilities is preferred. Ability to observe patients carefully for changes in condition through patient observation and patient monitoring techniques. Proficiency in using care equipment such as Hoyer lifts, patient transfer devices, blood pressure cuffs, thermometers, and other vital signs measurement tools. Excellent communication skills for engaging with residents respectfully while advocating for their rights; experience working with individuals with disabilities or geriatrics is a plus. Knowledge of HIPAA regulations to ensure confidentiality of resident information at all times. Ability to perform heavy lifting safely during patient transfers or when moving supplies; experience with manual handling is beneficial. Certification in CPR and First Aid to respond effectively during emergencies; experience working with individuals with neurodevelopmental or developmental disabilities is advantageous. Familiarity with home & community care practices including assisting clients with transportation needs and supporting their participation in community activities.
