Role:

Medical Social Worker (MSW) in Home Care.

Responsibilities:

Conduct psychosocial assessments, counseling, crisis intervention, and provide information/referral services. Collaborate with management to optimize resource utilization for patient outcomes. Assist patients and families in understanding and accepting care recommendations to promote social and health adjustment. Stay informed on community resources for social, emotional, and financial support. Identify and document barriers to healthcare transition, including clinical, psychosocial, and financial issues. Participate in audits to ensure compliance with laws and guidelines. Work with patients and support systems to develop and implement care plans. Perform initial psychosocial evaluations through patient interviews. Report suspected cases of abuse or neglect.

Qualifications & Skills:

Master's Degree in Social Work. Minimum 1 year of social work or mental health experience. Effective communication with diverse populations and adherence to NASW Code of Ethics. Knowledge of patient rights, laws (like HIPAA), and psychological principles. High emotional intelligence, empathy, and reasoning skills.

Preferred:

LSW license in Pennsylvania.
