Direct Support Professional East Stroudsburg, PA From $18 an hour - Part-time, Full-time From $18 an hour - Part-time, Full-time Job Overview Achievable Home Care, LLC is seeking a reliable and community-focused Direct Support Professional (DSP) to provide Community Participation Supports (CPS) to an adult with an intellectual disability. This role focuses on 1:1 community engagement, skill-building, and supporting independence through meaningful daily activities in the community. This is an excellent opportunity for someone who enjoys being active, social, and hands-on while making a real impact. Responsibilities Monitor and observe for any changes in health or behavior, documenting findings accurately in compliance with HIPAA regulations Follow an Individual Support Plan (ISP) meticulously, In order to strictly adhere to the Participant's personal goals, preferences, health needs, safety protocols, and service delivery, ensuring all care aligns with the participant's unique vision for an everyday life, fostering well-being, independence, and community inclusion through consistent, person-centered actions. Provide 1:1 Community Participation Supports in accordance with the individual's ISP and ODP guidelines Support safe and meaningful participation in community activities such as: Community outings and local shopping Leisure and recreational activities Exploring personal interests and hobbies Skill-building related to independence, communication, and decision-making Encourage choice-making and self-advocacy while maintaining professional boundaries Provide transportation within the community as needed Accurately document daily service notes and activities Follow all agency policies, participant plans, and ODP regulations Support participant's during outings or community activities to foster social engagement and community involvement Follow all safety protocols including CPR and First Aid procedures, ensuring readiness for emergencies at all times Skills Proven experience with persons having autism, developmental disabilities and/or senior citizens preferred. Strong knowledge of medication administration, vital signs monitoring, dementia care, Alzheimer's care, memory care, ADHD support, autism support, Service Notes. Ability to Communicate Progress Monitoring Excellent observational skills for patient monitoring and patient observation to detect early signs of health issues or behavioral changes Familiarity with HIPAA regulations to ensure confidentiality of Participant's information at all times Effective communication skills for collaborating with team members, families, and Program Specialist, Supports Coordinators, Behavioral Specialist, PCP's, Therapist, other Healthcare professionals Compassionate attitude combined with patience when working with individuals with developmental disabilities or cognitive impairments Experience with Community Participation Supports (CPS) Comfortable supporting individuals in public and community environments Organized, dependable, and proactive Knowledge of person-centered supports Qualifications High school diploma or GED required Valid driver's license, reliable vehicle, and auto insurance Ability to pass background checks (per ODP requirements) Experience supporting individuals with intellectual or developmental disabilities preferred Strong communication skills and a positive, patient demeanor Ability to work independently and in the community setting Why Join Achievable Home Care?
