Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report changes promptly Support medication management by administering prescribed medications following medical instructions and maintaining accurate documentation Perform patient transfers using safe methods like Hoyer lifts or transfer belts to ensure safety for both patient and caregiver Maintain detailed medical records and documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems in compliance with healthcare regulations such as HIPAA Provide companionship and emotional support to reduce feelings of isolation, especially for clients with dementia or Alzheimer's disease Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment aligned with home health care standards Follow infection control procedures and maintain patient confidentiality at all times during caregiving activities Support end-of-life or hospice care patients with dignity, compassion, and respect for their wishes Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred Proven ability to work effectively with geriatric patients and individuals with developmental disabilities or dementia Knowledge of medical terminology, catheter care, wound care, and basic life support (BLS) certifications such as CPR required or obtainable upon hire Experience operating EMR/EHR systems for accurate documentation of patient information and medical records Familiarity with home health regulations and safety protocols to ensure compliant caregiving practices Demonstrated ability to perform heavy lifting safely using appropriate equipment like Hoyer lifts when necessary Strong communication skills for client interaction and collaboration with healthcare teams Join us in providing compassionate home health care that empowers clients to live comfortably and independently.
