Job Overview We are seeking a dedicated and energetic Home Health Aide to join our team. In this vital role, you will provide compassionate in-home care to clients with diverse needs, including seniors, individuals with disabilities, and those requiring specialized memory or hospice care. Your efforts will promote independence, safety, and comfort while ensuring adherence to all home health care regulations. Duties Assist clients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs and reporting changes promptly Support medication administration and manage medical documentation accurately using Electronic Health Record (EHR) systems Provide specialized care for clients with dementia, Alzheimer's disease, or other cognitive impairments Operate patient transfer methods including Hoyer lifts and other assistive devices safely Maintain patient confidentiality in compliance with HIPAA regulations and ensure privacy during all care activities Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe environment Qualifications Proven experience working with individuals with developmental disabilities, geriatrics, or in assisted living settings Knowledge of medical terminology, catheter care, and basic life support (BLS) including CPR and first aid certification Familiarity with home health care assistance procedures, hospice care practices, and end-of-life support Experience using EMR/EHR systems for accurate medical documentation and records management Ability to lift heavy objects safely and perform patient transfers using appropriate techniques Strong communication skills for effective client interaction and team collaboration Valid certification in CPR and First Aid; additional certifications in dementia or Alzheimer's care are a plus Join us to make a meaningful difference in the lives of those we serve!
