Community Integration Counselor

At Transitional Options, Inc., we're dedicated to supporting adults with Intellectual and Developmental Disabilities (IDD) and Autism in living meaningful, independent lives. Our services are provided as 1:1 companion supports in individuals' own homes and communities. Most individuals do not require 24-hour support, and we offer longer shifts so staff can work fewer days while still maintaining full-time hours. As a Community Integration Counselor, you will play an important role in helping individuals thrive through community participation, work and leisure activities, volunteering, and developing independent living skills.

Essential Duties:

Provide transportation (company vehicles available or mileage reimbursement provided) Support individuals with personal care and daily activities (examples: yoga, walking their dog, scheduled company outings, cooking, gardening) Participate in staff meetings and training to foster growth and achieve goals Complete and maintain required documentation (THERAP system)
