Community-Based Support Position Overview Achievable Home Care, LLC is seeking a highly dependable, attentive, patient, and safety-focused Direct Support Professional to provide dedicated 1:1 support to an adult participant in the Springfield, PA area. This is a high-support assignment requiring consistent, active supervision and engagement throughout the entire shift. The selected DSP must be comfortable supporting an individual who requires significant assistance to safely and successfully participate in the community. This position is strictly community-based. The DSP will support the participant with safe, meaningful community participation while promoting dignity, choice, independence, and individual rights. Responsibilities Provide consistent 1:1 supervision and direct support during all scheduled service hours. Remain alert, attentive, and actively engaged throughout the shift. Support safe participation in community activities, recreation, errands, and other person-centered activities. Follow all individualized support plans, safety protocols, and approved behavioral support strategies as trained. Maintain appropriate proximity and supervision based on the participant's individualized needs. Use positive support, redirection, and de-escalation techniques when necessary and as trained. Monitor for changes that could affect the participant's health, safety, or well-being and follow agency reporting procedures. Encourage choice, independence, communication, and meaningful community participation. Protect the participant's privacy, confidentiality, dignity, and rights at all times. Complete accurate service documentation and progress notes. Promptly communicate significant concerns to appropriate agency personnel. The assigned DSP must provide active, consistent 1:1 attention and follow all individualized health, safety, behavioral, and supervision requirements provided during training. Specific participant information and individualized support requirements will only be shared with appropriate staff through Achievable Home Care's confidential onboarding and training process.
