Job Description

Overview We are currently seeking experienced Home Health Aides to assist clients in the Evenings, Overnights, and Weekends in and around Berks County, PA. Our compassionate team of Caregivers make a meaningful difference in the lives of individuals needing support with daily activities and personal care. This rewarding role offers the opportunity to provide essential assistance in clients' homes, ensuring their safety, comfort, and independence. As an In-Home Caregiver, you will be a vital part of a dedicated team committed to delivering high-quality, personalized care that promotes well-being and dignity. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting to promote independence and comfort. Support with medication administration and monitor vital signs to ensure health and safety. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Provide patient observation and monitor changes in health or behavior, reporting any concerns promptly. Help clients with mobility and transfers using proper techniques such as Hoyer lifts and patient transfer methods. Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment. Offer companionship and emotional support while respecting client preferences and privacy. Assist with transportation for medical appointments or community outings, ensuring safe travel. Follow care plans meticulously, documenting observations and activities in compliance with HIPAA regulations. Support clients with specialized needs including dementia, Alzheimer's, memory care, autism, developmental disabilities, or neurodevelopmental disorders. Skills Proven experience working in home health, assisted living, nursing homes, or group homes with elderly or disabled populations. Strong patient observation skills to detect changes in condition or behavior promptly. Knowledge of patient monitoring techniques including vital signs measurement and medication administration. Familiarity with care for individuals with dementia, Alzheimer's disease, or other neurodegenerative conditions. Ability to perform manual handling tasks safely using equipment like Hoyer lifts and patient transfer techniques. Excellent communication skills for effective client interaction and documentation of care activities. Basic first aid and CPR certification to respond confidently in emergencies. Experience working with individuals with autism or developmental disabilities supporting their unique needs. Competence in meal preparation tailored to dietary restrictions or preferences. Understanding of HIPAA regulations ensuring confidentiality of client information. Ability to handle heavy lifting safely when required for patient transfers or mobility assistance.
