Health Care Assistant (Medical Assistant)

Job Description

The Medical Assistant (MA) works as part of the health care team model and is responsible for providing outstanding patient care in the primary care/medical home model environment that fosters care collaboration with care team nurses, clinicians, behavioral health, front desk staff and community health workers in the design and provision of care as part of population health through a cohesive multidisciplinary care team. Under the direct supervision of a qualified provider/clinician, the MA ensures quality patient care is delivered that meets the needs of patients and their families. The MA is responsible for timely follow-up with referral tracking, patient call backs, scheduling of follow appointments during time of office visits, and appropriate electronic medical record (EMR) documentation. The individual displays responsible behaviors while demonstrating competency in all skills related to the performance of patient care. The MA functions under the direct supervision and authority of the provider when performing clinical tasks during patient care.
