Job Description

Job Summary We are seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to provide exceptional in-home care to clients across diverse populations. As a vital member of our healthcare team, you will deliver personalized assistance that promotes independence, safety, and well-being. Your role will involve working closely with patients, families, and healthcare professionals to ensure comprehensive support in daily activities and medical needs. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote comfort and dignity Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report any abnormalities promptly Administer medications accurately following prescribed schedules and medical protocols while maintaining detailed medical documentation Support patients with mobility and transfers using proper patient transfer methods and equipment like Hoyer lifts or gait belts to ensure safety Provide specialized care for individuals with dementia, Alzheimer's disease, or other cognitive impairments, including memory care techniques and behavioral support Maintain patient confidentiality by adhering to HIPAA regulations and safeguarding sensitive medical records Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment in compliance with home healthcare regulations Requirements Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) with current licensing in good standing Proven experience working with geriatric patients, individuals with disabilities, or those requiring hospice or end-of-life care Knowledge of medical terminology, EHR (Electronic Health Record) systems, and home health documentation standards Ability to work independently while communicating effectively with clients and healthcare teams Strong skills in patient observation, caregiving techniques, and emergency response including First Aid and CPR certification Physical ability to perform heavy lifting when necessary and assist with patient transfers safely Familiarity with home health care assistance tools such as Hoyer lifts and patient transfer methods Commitment to maintaining patient confidentiality in accordance with HIPAA guidelines Join our team to deliver compassionate home health care that transforms lives!
