Job Description
Job Overview Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their own homes. This position offers an energizing opportunity to make a meaningful difference in the lives of patients by assisting with daily activities, ensuring safety, and promoting independence. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality home healthcare services that enhance well-being and quality of life. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary Monitor and observe patients' conditions, recording vital signs and changes in health status accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Provide medication reminders and assist with medication administration following home health care regulations and medical documentation standards Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, end-of-life care, and working with individuals with developmental disabilities or disabilities Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients Maintain patient confidentiality in compliance with HIPAA regulations while documenting medical records and communicating effectively with healthcare teams Assist with patient transfers using proper techniques and equipment like Hoyer lifts or patient transfer methods to ensure safety for both client and caregiver Qualifications Proven experience working as a Home Health Aide or in assisted living, nursing home, or long-term care settings Knowledge of medical terminology, vital signs measurement, catheter care, medication administration, and basic life support (CPR/First Aid) certifications preferred Experience working with geriatric patients, individuals with disabilities, dementia or Alzheimer's disease is highly valued Familiarity with home health agency operations and home & community care standards is advantageous Strong communication skills for client interaction and team collaboration while maintaining patient confidentiality under HIPAA guidelines Ability to lift heavy objects safely and perform physical tasks associated with patient transfer methods and caregiving duties Certification in CPR/First Aid is preferred; ongoing training in hospice or palliative care is a plus Join us to provide essential home health services that truly make a difference!
