Job Summary We are seeking a compassionate and dedicated Personal Care Assistant to provide exceptional support and care to individuals in various settings, including residential homes, assisted living facilities, and community-based programs. In this vital role, you will assist clients with daily activities, monitor their health and safety, and foster a respectful, empowering environment. Responsibilities Assist clients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques. Monitor patients' vital signs, observe changes in health status, and document findings accurately in care plans while adhering to HIPAA regulations to protect patient privacy. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote proper nutrition. Support clients with medication administration under supervision, ensuring timely doses and proper documentation. Maintain a clean and safe environment by performing light cleaning duties and ensuring hygiene standards are met at all times. Provide personal transportation assistance for appointments or errands, supporting independence within the community. Engage in patient observation to identify signs of dementia, Alzheimer's disease, autism spectrum disorder, or other neurodevelopmental or developmental disabilities, reporting concerns promptly. Experience Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred. Demonstrated knowledge of patient care techniques including patient monitoring, first aid, CPR certification, and use of assistive devices like Hoyer lifts. Experience supporting individuals with dementia, Alzheimer's disease, memory care needs, or behavioral challenges associated with neurodevelopmental disorders. Familiarity with care plans development and implementation for residents with intellectual disabilities or developmental disabilities support needs. Proven ability to communicate effectively with clients and team members while respecting residents' rights and maintaining confidentiality under HIPAA guidelines. Hands-on experience assisting with ADLs such as grooming, feeding, mobility assistance, and personal transportation in a home or community setting. Ability to perform heavy lifting when necessary and ensure safety during patient transfers or manual handling tasks.
