Job Duties:

Assist individuals with personal care and self-help skills Support transportation and community-access needs Prepare meals and provide dining assistance Maintain a safe, clean, and comfortable living environment Monitor individuals' overall health, safety, and well-being Encourage independence and participation in daily activities Provide support within intermediate care facilities, community training homes, and supervised living programs Document services and daily activities in an electronic record-management system Communicate professionally with individuals, coworkers, and support teams
