Job Description
Duties and Responsibilities may include:
Work in client homes to provide routine personalized care Monitor the patient's health and maintain up-to-date records of the patient's physical and mental condition Discuss patient progress with the case manager or supervisor; adjust patient care plan as needed Direct the patient in prescribed range of motion exercises Assist the patient with personal care including bathing, dressing, grooming, and eating Help the patient in and out of bed, the bathroom, chairs and wheelchairs, and cars Perform simple housekeeping tasks: change bed linens, do laundry, tidy rooms Handle chores like grocery shopping and food preparation Make efforts to keep the patient engaged mentally through conversation, games, and other forms of entertainment Accompany the patient to doctor visits and other outings; assist with or provide transportationRequirements:
Must have at least 3 years of hands on patient care experience, be able to pass a pre-employment drug screening and SLED background check, 2-step TB test (if yours is not current, we will have one done), valid Driver's License and reliable transportation.
