Job Description

Job Summary Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. Whether working with seniors, individuals with disabilities, or those recovering from illness, you will be an essential part of a team committed to delivering exceptional home health care. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts or other assistive devices. Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates to track health status accurately. Administer medications following medical documentation protocols and ensure adherence to HIPAA regulations for patient privacy and confidentiality. Support patients with specialized care needs such as Alzheimer's disease, dementia, hospice care, or memory care by providing compassionate companionship and supervision. Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions, medical records, and care plans precisely. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment conducive to recovery or ongoing care. Work collaboratively with healthcare professionals to deliver comprehensive home & community care tailored to each individual's needs. Experience Prior experience in home health or home care settings is highly preferred, especially in assisted living facilities or nursing homes. Demonstrated ability to work with individuals with developmental disabilities or disabilities requiring specialized support. Knowledge of medical terminology and procedures related to long-term care, hospice, dementia, and Alzheimer's care. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as catheter care or medication administration are advantageous. Experience operating medical equipment like Hoyer lifts and taking vital signs accurately is essential. Familiarity with HIPAA compliance standards and medical documentation practices ensures high-quality patient care while maintaining confidentiality. Ability to work independently in live-in employment arrangements or flexible schedules while maintaining excellent patient care standards.
