Job Summary We are seeking dedicated and compassionate Home Health Professionals to provide exceptional care to clients in,, within Hamilton County. This vital role involves supporting individuals with diverse health needs, including seniors, individuals with disabilities, and those requiring specialized care such as Alzheimer's, dementia, or hospice services. The ideal candidate will demonstrate a strong commitment to patient-centered care, possess excellent communication skills, and have experience working in home healthcare environments. Responsibilities Provide comprehensive personal care, including assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support. Monitor and observe patients' health status by reporting changes to healthcare teams promptly. Medication reminders, accurately following medical documentation and home healthcare regulations. Support patients with medical procedures such as catheter care, hospice or end-of-life care. Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a safe and hygienic environment for clients. Skills Experience working with geriatric patients, individuals with developmental disabilities, or those requiring memory or dementia care. Familiarity with home health agency operations, home & community care practices, and relevant regulations. Excellent caregiving skills with a focus on patient observation, empathy, and respect for client dignity. Pay:
