Job Description
Job Summary Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and well-being. Your energetic approach and commitment to quality patient care will help foster independence and improve quality of life for those you serve. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those requiring end-of-life care, all within a supportive and dynamic environment. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' health status by taking vital signs such as blood pressure, pulse, respiration, and temperature Administer medications accurately following prescribed instructions while maintaining compliance with home healthcare regulations and HIPAA confidentiality standards Support patients with medical procedures such as catheter care, wound care, and other basic nursing tasks under supervision Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient information accurately and securely Assist with meal preparation and light housekeeping tasks like cleaning and laundry to ensure a safe and comfortable environment Employ proper patient transfer methods using equipment like Hoyer lifts or patient transfer techniques to ensure safety for both client and caregiver Qualifications Proven experience working as a Home Health Aide or in assisted living, nursing homes, or hospice settings Strong knowledge of medical terminology, medication administration, and vital signs monitoring Experience working with individuals with developmental disabilities, dementia (including Alzheimer's care), or geriatric patients Familiarity with home health care assistance protocols, including patient transfer methods and end-of-life care practices Ability to operate EMR/EHR systems efficiently for medical documentation purposes Certifications in First Aid, Basic Life Support (BLS), CPR are required; additional certifications such as hospice care or dementia training are a plus Excellent communication skills for effective client interaction and team collaboration Physical ability to perform heavy lifting when necessary and assist with patient mobility safely Join us in providing compassionate home health services that empower clients to live healthier, happier lives.
