You'll assist with daily living, personal care, and medication reminders while promoting independence, skill-building, and community inclusion. In this healthcare and community services role, you'll ensure safety, document progress, and collaborate with families and clinical teams. Responsibilities Provide direct support to individuals with developmental, physical, or behavioral needs in home and community settings Assist with daily living activities, including personal care, meal preparation, and medication reminders per care plans Support individuals in building independence, social skills, and participation in community activities Implement person-centered care plans and document services, progress notes, and incidents accurately Monitor health and safety, promptly reporting changes in condition or concerns to supervisors Transport individuals to appointments, programs, and community outings as needed Collaborate with families, healthcare providers, and team members to ensure consistent, quality care Follow all agency, state, and federal regulations, including confidentiality and rights of individuals Required Skills Personal care assistance Medication administration support (non-clinical) CPR and First Aid Behavioral support techniques Crisis de-escalation Patient/client documentation HIPAA compliance Infection control practices Safe transfer and mobility assistance Valid driver's license and safe driving record
