Job Description

Overview Join our compassionate team as a PRN Hospice Aide, where your dedication and caregiving skills will make a meaningful difference in patients' lives during their most vulnerable moments. This flexible role offers the opportunity to provide personalized, respectful care to individuals facing life-limiting illnesses, ensuring comfort, dignity, and emotional support. As a vital part of our hospice care team, you will work in home and community settings, delivering essential services that promote quality of life and peace of mind for patients and their families. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, grooming, dressing, and mobility to promote independence and comfort Monitor and document patient observations including vital signs, mental status, and overall well-being using Electronic Health Record (EHR) systems Support medication administration under supervision, ensuring proper documentation in compliance with HIPAA regulations Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques Assist with catheter care, wound care, and other medical procedures following established protocols Use equipment such as Hoyer lifts and other mobility aids safely to prevent injury and enhance patient comfort Perform light housekeeping duties like cleaning, laundry, meal preparation, and maintaining a safe environment in accordance with health standards Maintain accurate medical records and documentation of patient care activities while respecting patient privacy and confidentiality Experience Prior experience in hospice care or home health environments preferred; experience in assisted living or nursing homes is highly valued Knowledge of medical terminology and basic nursing skills such as vital signs measurement and medication administration Familiarity with EMR (Electronic Medical Records) or EHR systems for documentation purposes Experience working with individuals with disabilities or developmental challenges is a plus CPR certification required; additional certifications such as Alzheimer's or dementia care training are advantageous Long-term care experience including home & community care settings is highly desirable Strong caregiving skills with an energetic approach to providing compassionate patient-centered support Join us in delivering heartfelt care that truly makes a difference.
