In this vital role, you will provide personalized in-home care to geriatric individuals in Denton, TX and neighboring cities like Lewisville, Frisco, McKinney, Plano, Carrolton and Irving, promoting independence, comfort, and well-being. Your energetic approach and attention to detail will ensure that clients receive high-quality support while maintaining their dignity and safety. The ideal candidate must have at least one year of home health experience and a strong commitment to providing exceptional care to patients in their homes. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as wheelchair when necessary. Document patient observations and medical conditions accurately using Electronic Health Record systems to ensure seamless communication with healthcare providers. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment that complies with health standards. Maintain patient confidentiality and adhere to HIPAA regulations during all interactions and documentation processes. Skills Strong patient observation skills with the ability to recognize changes in condition or behavior promptly. Proven experience working with individuals with developmental disabilities, geriatrics, or in assisted living environments. Familiarity with home health care assistance, home & community care standards, and long-term care practices. Knowledge of basic life support including CPR certification. Ability to communicate effectively with patients, families, and healthcare teams to ensure comprehensive patient care.
