Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using appropriate patient transfer methods like Hoyer lifts or manual handling techniques. Monitor patients' vital signs and observe for any changes in health status, reporting findings accurately to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Support medication administration following care plans while adhering to HIPAA regulations to protect patient confidentiality. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed. Provide transportation assistance for appointments or community outings, ensuring client safety at all times. Document care activities, patient observations, and updates in compliance with healthcare standards such as resident's rights and documentation protocols. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred. Knowledge of patient observation techniques, dementia care (including Alzheimer's care), memory care, and hospice care practices. Ability to perform manual handling tasks including heavy lifting and patient transfers using equipment like Hoyer lifts. Certification in CPR and First Aid is required; additional certifications such as HIPAA training or medication administration are advantageous. Strong communication skills to effectively interact with clients, families, and healthcare teams. Experience working with individuals with disabilities, neurodevelopmental disorders such as autism or intellectual disabilities support is beneficial.
