Job Summary We are seeking a Bilingual (Spanish) compassionate and dedicated Home Health Aide to provide exceptional in-home care and support to clients with diverse needs. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' health status, recording vital signs such as blood pressure, pulse, temperature, and respiration Support medication administration following prescribed guidelines while maintaining accurate medical documentation in Electronic Health Record (EHR) systems Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and engagement activities Assist with transfers using patient transfer methods such as Hoyer lifts or gait belts, ensuring safety at all times Support clients with end-of-life care and hospice needs by providing compassionate emotional support and comfort measures Perform light housekeeping tasks like cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with home healthcare regulations Requirements Proven experience working as a Home Health Aide or in assisted living facilities, nursing homes, or long-term care settings Knowledge of HIPAA compliance to maintain patient confidentiality Certification in First Aid and Basic Life Support (BLS), including CPR training; additional certifications such as dementia or Alzheimer's care are a plus Ability to work independently while effectively communicating with clients and healthcare teams Physical stamina for heavy lifting and assisting with patient transfers; ability to stand, walk, bend, and lift for extended periods Experience working with individuals with disabilities or developmental challenges is highly desirable Familiarity with EMR/EHR such as Wellsky Kinnser for medical documentation and client records management Strong understanding of home health care assistance standards and home & community care best practices Apply online at www.delighthhc.com
