Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, recording vital signs and reporting changes promptly Administer medications and manage medical documentation in accordance with home healthcare regulations Support patients with specialized care needs such as Alzheimer's, dementia, or end-of-life care Operate Electronic Health Record (EHR) systems to document patient interactions accurately Use patient transfer methods safely, including Hoyer lifts and other assistive devices Maintain patient confidentiality and adhere to HIPAA guidelines at all times Prepare nutritious meals, assist with light housekeeping tasks such as cleaning and laundry, and ensure a safe living environment Communicate effectively with clients and their families to provide updates and address concerns Follow protocols for emergency response including first aid, CPR, basic life support, and wound care when necessary Requirements Proven experience working as a Home Health Aide or Caregiver in assisted living facilities, nursing homes, or home healthcare settings Knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring Experience working with individuals with developmental disabilities or geriatrics is highly preferred Ability to lift heavy objects safely and perform patient transfers using appropriate techniques such as Hoyer lifts or patient transfer methods Familiarity with home health care assistance regulations and documentation standards (EHR/EHR systems) Certifications in first aid, CPR, and basic life support are required; additional certifications in hospice or dementia care are a plus Strong communication skills to effectively interact with patients, families, and healthcare teams Ability to work independently while maintaining high standards of patient care and safety Valid driver's license and reliable transportation may be necessary for client visits Join us in providing exceptional home health care that empowers individuals to live comfortably and confidently.
