Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to ensure comfort and dignity Monitor patient health by observing vital signs, reporting changes, and maintaining detailed medical documentation using Electronic Health Record (EHR) systems Support medication management by administering prescribed medications following home healthcare regulations and safety protocols Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Assist with mobility and transfers using patient transfer methods and equipment such as Hoyer lifts or gait belts to prevent injury Perform light housekeeping duties including cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with health standards Document all patient interactions accurately while maintaining strict confidentiality in accordance with HIPAA regulations Qualifications Prior experience working with geriatric patients or in assisted living facilities is highly preferred Knowledge of medical terminology, catheter care, and end-of-life care practices is advantageous Certification in First Aid and Basic Life Support (BLS), including CPR training is required Experience operating EMR/EHR systems for medical documentation and record keeping is desirable Ability to lift heavy objects safely and perform patient transfers using appropriate techniques and equipment Strong communication skills for effective client interaction and team collaboration Familiarity with home health care assistance regulations and best practices for client safety and confidentiality Embark on a fulfilling career where your caregiving skills directly impact the quality of life for those you serve.
