Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor and record vital signs, patient observations, and overall health status Administer medications and follow care plans in accordance with healthcare protocols Prepare nutritious meals tailored to individual dietary requirements Provide companionship and emotional support to promote mental well-being Operate assistive devices such as Hoyer lifts and ensure proper safety procedures are followed Maintain a clean and safe environment through light housekeeping and infection control practices Document care activities accurately while adhering to HIPAA regulations Support clients during outings or community activities as needed Collaborate with healthcare professionals, family members, and social workers to coordinate comprehensive care Qualifications Prior experience in home & community care, assisted living, nursing homes, or hospice care preferred Knowledge of patient care techniques including Alzheimer's, dementia, memory care, autism spectrum disorders, and developmental disabilities Certification in First Aid and CPR required; additional certifications such as HIPAA training are advantageous Ability to work with sensitive information while maintaining confidentiality in compliance with HIPAA standards Strong observational skills for patient monitoring and recognizing changes in condition Physical ability for heavy lifting and assisting with mobility using equipment like Hoyer lifts or resident transfers Excellent communication skills for effective interaction with clients, families, and healthcare team members Compassionate attitude with a genuine desire to provide high-quality senior or long-term care services Valid driver's license preferred for community-based caregiving roles This position is ideal for individuals committed to providing empathetic support within a caring environment.
