Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' conditions, noting changes in health status and reporting findings accurately Administer medications and perform basic medical tasks such as taking vital signs and catheter care under supervision Support patients with meal preparation, feeding, light housekeeping, laundry, and cleaning to maintain a safe living environment Use electronic health record (EHR) systems to document patient information and medical records precisely and confidentially Employ proper patient transfer methods using equipment like Hoyer lifts or transfer belts to ensure safety during mobility assistance Provide emotional support and effective communication to foster trust and comfort for patients with dementia, Alzheimer's, or other cognitive impairments Requirements Proven experience working with individuals with disabilities, geriatrics, or in assisted living or nursing home settings Knowledge of home healthcare regulations, HIPAA compliance, and maintaining patient confidentiality Certification in First Aid and Basic Life Support (BLS), including CPR training Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts Familiarity with medical terminology, medication administration protocols, and vital signs monitoring Strong communication skills for effective client interaction and documentation of medical records Experience working with clients requiring hospice or end-of-life care is a plus Valid driver's license and reliable transportation preferred for community-based visits Join us to provide compassionate home health care that empowers patients to live healthier, happier lives.
