Job Overview We are seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to provide exceptional in-home care to clients across various age groups and health conditions. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document vital signs, medication administration, and changes in patient condition utilizing Electronic Health Record (EHR) systems while maintaining strict adherence to HIPAA regulations and patient confidentiality. Provide specialized care for individuals with dementia, Alzheimer's disease, developmental disabilities, or those requiring memory care or hospice support. Support patients with catheter care, wound management, and end-of-life care needs while ensuring safety during heavy lifting or transfers. Educate patients and family members on health management, medication routines, and safety procedures; communicate effectively with healthcare providers regarding patient status. Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment aligned with home healthcare regulations. Respond promptly to emergency situations by applying first aid or basic life support (CPR), ensuring the well-being of patients at all times. Experience Prior experience working as a Home Health Aide or in assisted living facilities is highly preferred. Demonstrated experience caring for geriatric patients or individuals with disabilities within home health or long-term care settings. Familiarity with medical terminology, medical documentation, and electronic health record (EHR/EHR) systems is essential. Proven ability to perform patient transfers using methods such as Hoyer lifts or other assistive devices safely and effectively. Experience working with individuals requiring dementia or Alzheimer's care, hospice services, or in memory care units is advantageous. Knowledge of home health care assistance regulations, HIPAA compliance, and best practices for patient personal care assistance. Ability to handle physically demanding tasks including heavy lifting while maintaining patient safety standards.
