Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using manual handling techniques or equipment like Hoyer lifts and patient transfer methods Monitor patients' health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals Support medication administration following HIPAA guidelines and care plans to ensure safety and compliance Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed Maintain a clean and safe environment by performing light cleaning duties, laundry, and ensuring infection control protocols are followed Provide companionship and emotional support, fostering positive communication and respecting residents' rights Support residents with cognitive impairments such as Alzheimer's disease or dementia through specialized memory care techniques Follow established procedures for patient observation, patient monitoring Collaborate with interdisciplinary teams to develop and implement personalized care plans that address each individual's unique needs Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred Knowledge of patient care practices including Alzheimer's care, dementia care, hospice care, and neurodevelopmental disorders support Familiarity with HIPAA regulations ensuring confidentiality of patient information Ability to perform manual handling tasks safely while assisting with heavy lifting or patient transfers using proper techniques Strong communication skills for effective client interaction and team collaboration Experience working with individuals with developmental disabilities or intellectual disabilities is a plus Demonstrated ability to follow detailed care plans and document observations accurately Compassionate attitude combined with patience, reliability, and a commitment to providing respectful resident-centered care Must
