Patient Care Assistant (PCA) Nexus Children s Hospital - Shenandoah | Shenandoah, TX Full-time | 12-hour day and night shifts available About Us Nexus Children s Hospital - Shenandoah is a 50-bed specialty hospital caring for children and adults with complex medical and behavioral needs. Our patients may be recovering from brain or spinal cord injuries, managing chronic illness, or receiving care for co-occurring medical and behavioral conditions. The Opportunity As a Patient Care Assistant , you will provide hands-on care and behavioral support as part of an interdisciplinary team. You will help patients with daily activities, share important observations with nursing staff, and contribute to a safe, structured, and compassionate environment. What You ll Do Assist patients with bathing, dressing, grooming, feeding, turning, positioning, mobility, and other activities of daily living. Obtain and document vital signs; promptly report changes in a patient s physical condition, behavior, or mental status. Perform point-of-care testing and specimen collection within your training and facility procedures. Complete assigned 15-minute observation checks and support one-to-one patient monitoring. Participate in bedside shift reports and share observations with the care team. Engage patients in therapeutic activities and group programming. Use approved de-escalation and crisis prevention techniques to support patient and staff safety. Assist licensed nursing staff with wound care support and minor clinical procedures, as directed. Maintain accurate documentation and follow infection prevention, privacy, and patient safety procedures. Help orient and support newer team members as needed. Qualifications High school diploma or equivalent required . At least one year of direct patient care experience in a hospital, behavioral health, rehabilitation, or long-term care setting required . Experience supporting patients with behavioral health conditions, cognitive impairments, or psychiatric disorders. Strong communication skills and the ability to provide compassionate care while maintaining professional boundaries. Ability to use an electronic health record and document care accurately. BLS certification from the American Heart Association or American Red Cross required , valid for at least six months from the date of hire. Ability to complete required de-escalation training within 30 days of hire. CNA training or certification and additional behavioral health or rehabilitation training are preferred.
