In this vital role, you will empower clients to maintain their independence, enhance their quality of life, and receive personalized assistance with daily activities. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patients' health status by observing vital signs and overall well-being, reporting any concerns promptly Support medication administration following care plans and HIPAA regulations to ensure patient privacy and safety Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary Use proper patient transfer methods, including Hoyer lifts and other manual handling techniques, to ensure safe mobility Maintain a clean and safe environment through regular cleaning tasks and adherence to infection control protocols Provide emotional support and effective communication to foster trust and comfort for clients with dementia, autism, or other disabilities Requirements Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders is advantageous Ability to observe patients carefully and document changes accurately for care plans Experience with patient monitoring tools such as vital signs measurement and patient transfer methods Certification in CPR and First Aid is required; additional certifications like HIPAA training are a plus Physical ability to perform heavy lifting, assist with manual handling tasks, and support patient transfers safely Strong communication skills for effective interaction with clients, families, and healthcare teams Join us in delivering compassionate care that truly makes a difference!
