Job Overview We are seeking a compassionate and dedicated Personal Care Assistant to provide vital support and care to individuals in various settings, including home care, assisted living, nursing homes, and group homes. Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using appropriate patient transfer methods like Hoyer lifts or manual handling techniques Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately while respecting residents' rights and privacy under HIPAA regulations Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary Support patients with medication administration following prescribed care plans and ensure proper documentation of medication schedules Provide companionship and emotional support to promote mental well-being, especially for those with dementia or neurodevelopmental disorders like autism or intellectual disabilities Maintain a clean and safe environment through light cleaning duties and adherence to infection control protocols Assist with transportation for appointments or community outings using personal transportation assistance or mobility aids as required Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred Knowledge of patient observation techniques and ability to recognize signs of health deterioration or distress Certification in First Aid and CPR is required; ongoing training in hospice care or Alzheimer's/dementia/memory care is a plus Familiarity with care equipment such as Hoyer lifts, patient transfer methods, and vital signs monitoring tools Ability to handle heavy lifting safely and perform manual handling tasks efficiently Strong communication skills for effective client interaction and documentation of care activities Experience working with individuals with disabilities or neurodevelopmental disorders is advantageous Commitment to maintaining resident's rights and confidentiality in accordance with HIPAA standards Flexibility to work various shifts including days, evenings, weekends, or overnight shifts as needed Join us in providing exceptional person-centered care that enhances quality of life!
