Full Job Description Overview Platt's Family Residential Services Job Description Direct Support Professional Purpose:
The position of Direct Support Professional is under the direct supervision of the Residential Supervisor. The Direct Support Professional provides active treatment and training to individuals with disabilities receiving services in a residential setting. Active treatment, training and support services are provided to reflect the identified target goals on the Individual Service Plans. At all times compliance with the Virginia Department of Behavioral Health and Developmental Services and the Office of Licensure and Recertification required. Major Duties and Responsibilities 1. Provide services and supports as identified in each individual's Individualized Service Plan. 2. Interact with all individuals and staff with appropriate voice tone, language, gestures and physical movements in accordance with Platts Family 3. Services Human Rights Policies and Procedures. 4. Create a living environment consistent with existing policies, procedures and the concept of normalization. 5. Provide training and support in the activities of daily living: bathing, dressing, eating, cleaning, etc. 6. Provide training and support with social interaction skills: community integration, communication, writing, leisure activities, respect for self and others. 7. Provide opportunities for individuals to have choices and make decisions in daily living. 8. Provide transportation for individuals during community outings/activities and scheduled appointments when needed. 9. Attend work as scheduled which includes mandatory training activities and staff meetings. 10. Provide support to individuals in community settings. 11. Complete Programming and Recreation ISP documentation, progress notes and all other documentation required in each individual ID Notebook. 12. Complete Behavior Narratives and Accident/Incident Reports in a timely manner 13. Complete time sheets and leave requests in a timely manner. 14. Ensure a healthy, clean and safe environment, and report any safety concerns to management immediately. 15. Provide behavior support services as identified on the Positive Behavior Support Plan. 16. Maintain Person Centered goals for individuals supported by Platts Family Residential Services LLC. 17. Exhibit an overall spirit of cooperation to ensure a positive and productive environment for individuals and staff. 18. Complete other duties as assigned. 19. Ensure that all individuals' clothing and personal items are cared for, properly stored and accounted for. Qualifications 1. High school diploma, or general education diploma (GED) (CNA or higher level of education preferred) and six (6) months experience working with children. 2. Must be 21 years of age or older. 3. Must have a valid Virginia Driver's License and good DMV record. 4. Effective oral and written communication skills. 5. Good organizational skills. 6. Ability to work as a member of an interdisciplinary team. 7. Ability to handle emergency situations effectively utilizing crisis intervention techniques when appropriate.
Language/Math/Reasoning Ability:
Ability to read and comprehend simple instructions, correspondence and memos. Ability to write simple documentation. Ability to effectively present information in one-on-one situations, families and other HHP employees. Ability to add and subtract two digit numbers, multiply and divide. Physical Demands:
While performing the duties of this job, the employee is regularly required to stand, walk, reach, stoop, crawl and kneel. Some individuals might become physically aggressive and require the employee to physically redirect. This requires physical flexibility and endurance, emotional calmness and the ability to follow the individuals' Crisis Plan, implementing the approved intervention techniques and adhering to policy. The employee might also be required to lift and/or move up to fifty (50) pounds. It also might be required to stand for an undetermined extended period of time while supporting the individuals.
Work Environment:
While performing the major duties of this position, employees are frequently exposed to outside weather conditions. The employee is also frequently exposed to wet and/or humid conditions during bath times. The noise level is usually moderate, however it can increase in some situations.
