Job Overview We are seeking a dedicated and compassionate Direct Support Professional to provide exceptional care and assistance to individuals with diverse needs. The successful candidate will be proactive, attentive, and skilled in managing a range of patient needs with professionalism and empathy. Responsibilities Assist residents with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and mobility support using appropriate patient transfer methods such as Hoyer lifts. Monitor residents' vital signs and observe patient behavior to identify changes or concerns that require intervention. Administer medication following prescribed care plans while maintaining strict adherence to HIPAA regulations and confidentiality standards. Prepare nutritious meals in accordance with dietary guidelines and individual preferences, ensuring proper food safety practices. Support residents during medical appointments or community outings by providing transportation assistance and supervision. Implement individualized care plans tailored to each resident's needs, including dementia care, Alzheimer's care, memory care, or support for individuals with autism or developmental disabilities. Maintain a clean and safe environment by performing routine cleaning tasks and adhering to health and safety protocols. Document all care activities accurately in compliance with healthcare regulations and organizational policies. Foster respectful communication with residents and their families to promote transparency and trust. Participate in ongoing training related to patient monitoring, first aid, CPR, manual handling, and other relevant caregiving skills to ensure high-quality support. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Demonstrated knowledge of patient care practices including medication administration, vital signs monitoring, and patient personal care assistance. Familiarity with dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders such as autism is preferred. Experience supporting individuals with intellectual or developmental disabilities supports successful performance in this role. Proven ability to work effectively with diverse populations including seniors and individuals with disabilities requires strong communication skills. Hands-on experience using patient transfer equipment such as Hoyer lifts or other manual handling devices is advantageous. Knowledge of HIPAA regulations ensures confidentiality when managing sensitive health information.
