Responsibilities Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques like Hoyer lifts. Monitor patients' health by taking vital signs, observing for changes in condition, and reporting findings to healthcare professionals promptly. Support medication administration following HIPAA guidelines and care plans to ensure safe and accurate delivery of treatments. Prepare nutritious meals tailored to individual dietary needs, ensuring proper food safety and cleanliness standards are maintained. Provide personal transportation assistance for appointments, errands, or community outings to promote independence and social engagement. Maintain detailed documentation of patient care activities, observations, and progress notes while respecting patient privacy and confidentiality. Assist with specialized care such as Alzheimer's care, dementia support, memory care, autism support, and neurodevelopmental disorder assistance to meet diverse client needs. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly valued. Knowledge of patient observation techniques, patient monitoring, and the use of equipment like Hoyer lifts or patient transfer methods. Familiarity with care for individuals with developmental disabilities, intellectual disabilities support, geriatrics, or hospice care is preferred. Certification in CPR and first aid is required; additional certifications such as medication administration or HIPAA training are advantageous. Ability to perform heavy lifting safely and assist with manual handling tasks while maintaining proper body mechanics. Excellent communication skills for effective client interaction and collaboration with healthcare teams.
