Job Description

Make Every Day Matter InterGrated Health Care Consultants is looking for caring, dependable, and energetic Community Coaches to support adults with intellectual and developmental disabilities as they live, learn, work, and enjoy their communities. As a Community Coach, you'll build meaningful relationships while helping individuals develop independence, confidence, and life skills through community outings, volunteer opportunities, recreation, employment support, and everyday experiences. Every day is different, and every day is an opportunity to make a lasting impact. Responsibilities Provide one-on-one support to adults with intellectual and developmental disabilities in community settings. Assist individuals in achieving goals outlined in their Individual Support Plan (ISP). Support participation in community activities, recreation, volunteer work, and employment opportunities. Encourage independence while providing assistance only as needed. Provide transportation to and from community activities using your personal vehicle or company vehicle, as assigned. Assist with communication, social skills, budgeting, shopping, meal planning, and other daily living skills. Complete accurate and timely documentation of services provided. Communicate effectively with supervisors, families, residential staff, and other members of the support team. Maintain a safe, respectful, and positive environment at all times. Qualifications High school diploma or GED required. Experience working with individuals with intellectual or developmental disabilities is preferred but not required. We provide training! Compassionate, patient, dependable, and professional. Excellent communication and interpersonal skills. Valid driver's license with a clean driving record. Reliable transportation and proof of insurance. Ability to pass required background checks. CPR/First Aid certification (or willingness to obtain).
