Gates of Beautiful is looking to hire compassionate and experienced Direct Support Professionals for our Adult Day Support Program for adults with various developmental disabilities! We're willing to train those who have no experience, and who are dedicated in supporting the individual's growth. We offer; Two weeks' paid vacation a year Flexible working hours Comprehensive training and on the job training Closed most Federal Holidays with pay
Job Summary:
This position is responsible for creating, supporting and leading assigned individuals in their daily programming in the specific areas of social, life and leisure skills, areas of academics, personal care assistance, as well as recreational activities. Assist in the development and implementation of Personal Centered Individual Service Plans (ISP), including but not limited to social, community inclusion, communication, behavior, and skill development. Ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, as well as other funding sources and company policies and procedures.Day Support Job Description :
- Provide direct support to Individuals in accordance with written ISP outcomes.
- Monitors and documents daily on individuals progress toward written ISP outcomes.
- Maintains and follows a daily schedule of training and meaningful activities.
- Performs morning and afternoon transition duties including but not limited to transportation assistance, personal care, and setting up of activities.
- Upholds all individual's Human Rights and serves as an advocate for consumers
- Completes and maintains all required GOB and licensure training.
- Transport and support individuals while participating in community activities such as fitness, visual arts, recreational classes, volunteerism, shopping for cooking classes, dining, educational tours, museums, festivals, etc.
- Assist individuals with establishing relationships in the community, as appropriate to fully integrate and use the community as a learning environment.
- Implement Day Support services that are in full compliance with Home and Community-Based Services (HCBS) Settings Regulations
- Communicate with family members, guardians, authorized representatives, residential providers and support coordinators regarding individual's illness, incidents, and progress at the direction of the Program Manager or designee.
- Keep team members apprised of any concerns.
Additional Responsibilities :
- Responsible for a safe, clean, and orderly environment.
- Immediately report any unsafe conditions, medical concerns to the Program Manager or designee.
- Report any allegations of abuse, neglect, and exploitation according to Gates of Beautiful policies and procedures.
