Home Health Aide Job Type:

PRN / Part-Time Job Description Summary The Home Health Aide (HHA) is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist. The HHA provides personal care and supportive services based on the patient's needs and established plan of care. The HHA is responsible for observing and reporting changes in the patient's condition, accurately documenting care provided, promoting patient safety, and supporting quality and continuity of care. Essential Job Functions and Responsibilities Responsibilities include, but are not limited to: Provide personal care services, including: Bathing and personal hygiene Oral hygiene Shampoos Back rubs Skin care to help prevent breakdown Assistance with dressing and undressing Assistance with toileting Changing bed linens Assist with ambulation, positioning, and exercise according to the plan of care. Perform range-of-motion exercises and other simple procedures as ordered and within the HHA's demonstrated competency. Plan and prepare nutritious meals according to the patient's needs and plan of care. Assist with feeding when necessary. Assist patients with self-administration of medications, as permitted by the patient's plan of care and applicable requirements. Take and document temperature, pulse, respiration, and blood pressure when ordered and when the HHA has completed and demonstrated the appropriate competency. Assist with maintaining a clean, safe, and orderly patient living environment as appropriate. Perform patient laundry and other light household tasks directly related to the patient's care, as appropriate. Follow safety procedures and use patient care equipment properly and safely. Observe and report the patient's condition and any significant changes to the assigned nurse or therapist. Accurately document care provided and observations in accordance with agency requirements. Follow the organization's policies, procedures, documentation standards, and professional conduct expectations. Maintain patient dignity, privacy, and confidentiality at all times. Perform other job-related duties as assigned. Position Qualifications Current CNA, PCA, or HHA certification/license as required in the Commonwealth of Virginia. Must be at least 18 years of age. Ability to read and follow written instructions and accurately document care provided. Ability to work independently with limited direct supervision. Demonstrates compassion and empathy for individuals who are ill, injured, elderly, frail, or impaired. Current CPR certification. Demonstrates professionalism, tact, patience, and good personal hygiene. Valid driver's license and reliable transportation. Automobile must be properly insured in accordance with organization requirements and maintained in good working condition.
