Job Overview We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA) to join our vibrant home care team. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document patient vital signs, such as blood pressure, pulse, temperature, and respiration. Support patients with medication reminders following medical protocols while maintaining strict adherence to HIPAA regulations and patient confidentiality standards. Observe and report changes in patient condition, including behavioral or physical health concerns related to dementia care, Alzheimer's care, or end-of-life care. Assist with catheter care, and basic first aid or CPR when necessary. Provide emotional support and effective communication tailored to patients with developmental disabilities or those requiring memory care. Maintain a clean and safe environment by assisting with laundry, cleaning duties, meal preparation, and ensuring infection control standards are met in compliance with home health care regulations. Qualifications Valid certification as a CNA or PCA with current state licensing or registration. Proven experience working in assisted living facilities, nursing homes, hospice care, or home healthcare settings. Strong knowledge of medical terminology, patient care techniques, and home health agency procedures. Experience working with geriatric patients or individuals with disabilities, including dementia or Alzheimer's disease, is highly preferred. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts or other mobility aids. Familiarity with EMR/EHR systems for accurate documentation of patient records and medical history. Excellent communication skills for effective client interaction and team collaboration while maintaining patient confidentiality under HIPAA guidelines.
