Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs and reporting changes promptly Administer medications and perform catheter care following medical guidelines and client care plans Support clients with meal preparation, feeding, and maintaining proper nutrition Document patient care activities accurately using Electronic Health Record (EHR) systems and maintain confidentiality in accordance with HIPAA regulations Assist with patient transfers using proper methods and equipment like Hoyer lifts or transfer belts Provide companionship and emotional support to reduce feelings of loneliness or anxiety Follow home healthcare regulations and safety protocols during all caregiving activities Requirements Proven experience working with individuals with developmental disabilities, dementia, Alzheimer's disease, or in geriatrics and hospice care settings Knowledge of medical terminology, medication administration, and basic life support (CPR/First Aid) certifications preferred Experience operating EMR systems for medical documentation and maintaining accurate records Ability to perform heavy lifting and assist with patient transfers safely using appropriate equipment Strong communication skills for effective client interaction and team collaboration Valid certification in first aid and CPR; additional certifications in hospice or end-of-life care are a plus Familiarity with home health care assistance, home & community care experience, and working within home healthcare regulations Demonstrated ability to maintain patient confidentiality while providing compassionate care in a professional manner Able to speak multiple languages such as: Urdu, Punjabi, Hindi, Pashto, English Join us to be part of a dedicated team committed to delivering exceptional home health services.
