Job Summary We are seeking a compassionate and dedicated PT Home Health Care Aide to provide exceptional in-home care to clients across diverse settings. Assist patients with Activities of Daily Living (ADLs), ensuring comfort and dignity. Support medication administration with reminders, following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques. Assist with mobility and patient transfer methods using equipment such as Hoyer lifts or transfer belts to ensure safety for both patient and caregiver. Perform light housekeeping duties including cleaning, laundry, meal preparation, and maintaining a safe environment for clients. Collaborate effectively with healthcare teams by documenting medical records, communicating client needs clearly, and adhering to all home health agency policies. Qualifications Proven experience working with individuals with disabilities and/or assisted living environments is highly preferred. CPR training. Familiarity with client record management. Ability to perform modest lifting when necessary and utilize patient transfer methods safely. Strong communication skills to effectively interact with patients, families, and healthcare professionals. Prior experience in geriatrics or hospice care is advantageous; familiarity with home & community care standards is a plus.
