Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring all medical documentation complies with healthcare regulations. Support medication administration under supervision, adhering to HIPAA guidelines and maintaining patient confidentiality at all times. Observe patients closely for changes in condition or behavior, particularly in memory care, dementia care, Alzheimer's care, and geriatrics populations. Document patient progress and interactions using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems with precision and clarity. Provide emotional support and companionship to patients, especially those receiving hospice or end-of-life care, fostering a compassionate environment. Assist with light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living space aligned with home health care standards. Skills Strong knowledge of medical terminology and home healthcare regulations to ensure compliant patient care practices. Proven experience working with individuals with developmental disabilities or disabilities requiring specialized caregiving. Excellent patient observation skills to detect subtle changes in condition or behavior promptly. Familiarity with assistive devices like Hoyer lifts and patient transfer techniques to promote safety during mobility assistance. Ability to work effectively within a team while maintaining high standards of patient privacy and confidentiality under HIPAA guidelines. Competence in vital signs measurement, medication administration, catheter care, and basic life support (BLS) including CPR. Experience working in assisted living facilities, nursing homes, or home health agencies is highly desirable. Demonstrated caregiving skills for geriatric patients across diverse settings such as hospice or long-term care environments. Capable of heavy lifting when necessary to ensure safe handling of patients or equipment. Strong communication skills for client interaction and collaboration with healthcare professionals to deliver comprehensive care.
