A Home Health Aide helps individuals who are disabled, elderly, or chronically ill with daily tasks, such as dressing, washing, and light housekeeping. They can care for patients in their homes or other healthcare settings, and may perform basic medical care under the direction of a nurse or other medical professional. These roles typically require more comprehensive agency-approved training programs.
The Direct Support Professional located at the Geriatric Transition Center will provide direct services to individuals with mental, behavioral, and substance abuse issues within a Transition Center unit that provides residential stabilization as well as medically-managed services. Provides direct care support and instruction, as needed, and assist treatment team members in implementing individualized recovery plans. Completes household duties to ensure cleanliness, safety, and compliance with licensure regulations.
ESSENTIAL FUNCTIONS
(Maximum of Eight) 1. Provide individualized and group services to adults meeting criteria for the Geriatric Transition Center in order to promote recovery and meet the program goal of reducing reliance on more restrictive inpatient services. Assist in implementing Individualized Recovery Plans, including support, assistance with independent living skills, social skills, personal hygiene, household skills, cooking, nutrition and meal planning education and assistance, and transportation to recreational groups and activities. 2. Ensure a clean and safe environment by performing such task as fire drills, fire extinguisher checks, house cleaning, grocery shopping, meal planning, and preparation. 3. Participate in the activities of a self-directed team, and as needed, assigned, or scheduled, to attend meetings, and trainings events. 4. Maintain, administer, and document medications in accordance with written physician's orders and regulatory standards, and maintain current certification in the Agency's approved medication training program. Observe persons for potential side effects of medications, and notify appropriate staff as necessary. Administer basic first aid as needed and maintain current certification in CPR, First Aid, and other required training. 5. Appropriately intervene in crisis situations and emergencies, making referrals and seeking consultation from other team members as needed. 6. Participate with team members in the formulation of person-centered recovery planning. This may also include developing goals, objectives, plans, and data sheets in order to provide accurate and comprehensive documentation. Minimum Requirements
QUALIFICATIONS
Ability to interact with people in a supportive and therapeutic manner Knowledge of the nature of severe mental illness and substance use disorders. Good oral and written communication skills Ability to assist in developing and implementing service plans Skills in data collection Ability to work as part of a team Must be able to work independently and with supervision available at broadly-spaced intervals Ability to pass behavioral modification, medication, and other required trainings, with or without accommodation.
EXPERIENCE/EDUCATION REQUIRED
High school or equivalent. Valid driver's license with a safe driving record.
