Sevita is seeking a compassionate Direct Care Professional to support individuals of all ages and abilities in community and home-based settings. In this role, you'll assist with daily living activities, implement care plans, promote independence, and provide emotional support. Responsibilities Provide direct support with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and feeding Assist with mobility, transfers, and the safe use of adaptive equipment as needed Support individuals in following person centered care plans and behavior support plans Prepare simple, nutritious meals and assist with eating according to dietary needs Accompany individuals to appointments, community activities, and recreational outings Encourage skill-building, social engagement, and independence in home and community settings Monitor and document changes in physical, emotional, or behavioral status and report concerns promptly Follow medication assistance procedures as permitted and directed by nursing or supervisory staff Maintain a clean, safe, and organized living environment, following infection control and safety protocols Complete required documentation and electronic records accurately and in a timely manner Participate in required training, team meetings, and professional development activities Collaborate respectfully with families, guardians, and team members to support individual goals Required Skills Personal care and activities of daily living (ADL) support Basic knowledge of health and safety practices Behavior support and de escalation techniques (entry level) Observation and documentation of client status Use of electronic health or service documentation systems Time management and reliability in shift-based work Clear verbal and written communication Infection control and universal precautions Safe transfer and mobility assistance Basic meal preparation and nutrition awareness Professional boundaries and confidentiality (HIPAA awareness)
