18-23/hour Are you a caring and dependable Certified Home Health Aide (HHA) looking to make a meaningful difference? Join Preceptor Home Health & Hospice, a trusted provider dedicated to delivering high-quality, patient-centered care. In this role, you will provide essential personal care and support to patients while being a valued member of the interdisciplinary care team. What You'll Do As a Home Health Aide, you will provide hands-on support to patients in their homes, helping ensure comfort, safety, and dignity.
Responsibilities include:
Assisting with personal care needs such as bathing, grooming, dressing, and toileting Supporting skin care and hygiene to prevent breakdown Preparing meals and assisting with feeding when needed Performing light housekeeping and maintaining a clean, safe environment Assisting with mobility, ambulation, and range-of-motion exercises Taking and recording vital signs as directed and within scope Assisting with self-administration of medications (as allowed) Observing and reporting changes in patient condition to the care team Documenting care and maintaining accurate records Participating in care team meetings and case conferences Supporting patient safety and proper use of equipment Participating in on-call, weekend, and holiday rotations What We're Looking For Completion of a state-approved Home Health Aide training and competency program Listed in good standing on the state nurse registry Minimum age of 18 years Ability to follow written care plans and document services provided Ability to work independently with minimal supervision Compassion, patience, and strong interpersonal skills Commitment to maintaining patient dignity and confidentiality Valid driver's license with a reliable, insured vehicle Why Join Us?
