Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) systems Administer medications and treatments as prescribed, ensuring proper documentation in compliance with home healthcare regulations Support patients with medical procedures such as catheter care, vital signs measurement, and basic wound care Aid with meal preparation, light housekeeping, laundry, and maintaining a clean, safe environment Provide companionship and emotional support to clients with dementia, Alzheimer's disease, or other cognitive impairments Follow patient transfer methods safely using equipment like Hoyer lifts or other assistive devices while adhering to patient safety protocols Maintain strict confidentiality of patient information in accordance with HIPAA regulations and uphold patient rights at all times Skills Proven experience working with individuals with developmental disabilities, geriatrics, or in hospice care settings Strong knowledge of medical terminology, medication administration procedures, and vital signs monitoring Skilled in patient observation, documentation of medical records, and using EMR/EHR systems efficiently Ability to assist with complex care needs such as end-of-life support or dementia care with compassion and professionalism Familiarity with home health care assistance techniques including patient transfer methods and use of mobility aids like Hoyer lifts Excellent communication skills for effective client interaction and collaboration with healthcare teams Knowledge of home health regulations, safety standards including first aid, CPR, and basic life support (BLS) certifications preferred Capable of heavy lifting when necessary and maintaining a safe environment for both clients and staff Join us if you are passionate about providing exceptional patient care in a supportive environment.
