Position Summary:
Licensed Practical Nurse (LPN) PRN We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team on a PRN (as-needed) basis. The LPN provides high-quality nursing care to residents while working closely with Registered Nurses, physicians, and the interdisciplinary team to promote each resident's health, comfort, and well-being in our long-term care and rehabilitation facility. Qualifications Current Licensed Practical Nurse (LPN) license Current BLS/CPR certification required Strong clinical and assessment skills Experience in long-term care or rehabilitation preferred, but not required Excellent communication and interpersonal skills Compassionate, dependable, and resident-focused Ability to work effectively in a team environment Key Responsibilities Provide direct nursing care to residents in accordance with individualized care plans. Administer medications and treatments as prescribed. Monitor residents' conditions and promptly report changes to the RN or physician. Accurately document assessments, treatments, and resident responses. Assist with admissions, discharges, and physician orders. Collaborate with the interdisciplinary team to provide exceptional resident care. Supervise Certified Nursing Assistants (CNAs) and support quality resident care. Maintain compliance with all state, federal, and facility policies and regulations. Promote a safe, clean, and compassionate environment for residents and families.
